Service Description
This booking is only for 6 weeks, 10 weeks, 14 weeks, 9 month, 6 years & 12 years. Phone pharmacy first before booking 021 6969820 to check availability of stock to avoid disappointment! Please call first to check availability. Also expect a call in the morning to confirm appointment and stock before coming to clinic, should you by chance not receive a call please call pharmacy at 021 6969820 to confirm appointment and stock. Bring clinic book, without we won't be able to administer vaccination. Please note in certain indications baby vaccinations can be denied, due to illnesses or drug interactions. The pharmacy is under no obligation to continue baby vaccinations or initiate baby vaccination if it is found to be unsafe for the patient. Baby should not have diarhoea, runny nose, coughing or any infection or on any antibiotic before coming to the clinic. Please note the nurse/prescribing pharmacist will do a thorough consult first in clinic before giving the vaccination. If baby/child is found to be sick then we won't do vaccination, but will keep your form until child is better. We might suggest medication until child is better or refer. Baby vaccination Consultation will automatically stay a normal consult since time was spent in the clinic. Unfortunately no refunds on baby vaccinations will be given.
Cancellation Policy
-Although we we will do our utmost best to help you at your allocated timeslot the possibility exist that you might have to wait for your consultation. -Please alert the dispensary if you need to be seen to urgently. -Unfortunately no refunds on consultation. We will assist in rescheduling or treating if patient should be unfit for any reason to get his/her vaccination/injection or specified service. -Consultations, Vaccinations & any medication are done by either prescribing pharmacist or nurse in the clinic. Please note pharmacy have the right to deny admission or administration of vaccine, consults and medication due to illness, contraindications, allergies and drug interactions. No refunds on consultations or medication. -Please note sick notes/visitation/referral notes are given upon discretion of prescriber. Sick note is not mandatory given after paying a consultation only if the prescriber deem it necessary for a patient. -Sick and visitation notes will be emailed to the workplace and to the patient. -We will contact you and inform you if there are any changes to the booking calendar or if nurse is sick or unavailable. Please also call to double check. -We will also try our utmost best to have enough stock in anticipation of booking, please also call to double check stock of injections/vaccinations. -If an emergency should arise in the clinic and we have to help with the code red patient please be patient we have to help a code red patient first.
